Ultherapy vs Thermage in Korea: Which One Is Right for You? (2026)
By Dr. IM JUNHYUNG · Updated
- They work on different layers: Ultherapy reaches the SMAS at about 4.5 mm; Thermage heats the dermis evenly.
- Both are usually back to normal the same day, and flying the next day is typical.
- The main change builds over 2–3 months; both are commonly repeated about once a year.
- More shots is not automatically better — the plan by depth and area matters more.

Two different layers
Think of the face in layers. The skin (dermis) sits on fat, and under the fat lies the SMAS — the fibrous layer a surgeon tightens in a facelift. Sagging usually starts when the SMAS and the ligaments that anchor it loosen. Crepey, thin-looking skin is a change in the dermis itself.
Ultherapy and Thermage are the two best-known non-surgical options in Korea because each one targets a different part of that picture. They are not two brands of the same thing, so the useful question is not which one is stronger, but which layer has changed.
What Ultherapy does best
Ultherapy focuses ultrasound energy at set depths — 4.5 mm for the SMAS, 3.0 mm for the deep dermis and 1.5 mm for the superficial dermis — creating tiny thermal points without cutting the skin. Live imaging shows the layers on screen, so each line goes where it is meant to and bone is avoided.
It is FDA-cleared for lifting the brow, the skin under the chin and on the neck, and for improving lines on the chest. In practice it is the first choice for jowls and a jawline that has lost its edge.
The common mistake is chasing a high shot count. On a slim face, too much energy at the wrong depth can make the cheeks look hollow. We start from the layer, not the number.
What Thermage does best
Thermage FLX uses monopolar radiofrequency to heat the whole dermis evenly. Collagen contracts and then rebuilds over the following months, so the skin feels firmer and fine lines soften. Each pulse comes with cooling and vibration to keep it comfortable.
It suits skin that has thinned or lost its spring more than skin that has dropped. A dedicated eye tip can treat the upper and lower eyelid skin with eye shields in place — an area that ultrasound lifting does not treat.
A one-minute mirror check
These are rough self-checks, not a diagnosis, but they help you describe what bothers you:
- Lift test. Place your fingertips just in front of your ears and lift gently upwards. If the jawline sharpens and the jowls disappear, deeper sagging is a big part of the picture — Ultherapy’s territory.
- Lie-down test. Look in a hand mirror lying on your back. If your jawline looks clear when lying down but not when sitting up, it is sagging rather than volume.
- Texture check. Look at your cheek in side light while smiling gently. Fine, crepe-like creasing or skin that looks thin points to the dermis — Thermage’s territory.
If both apply, you probably have both changes; most people over 35 do. At consultation we check skin thickness, the depth of sagging and facial fat in person before choosing, and if neither treatment will help much, we will say so.
When both make sense
Many faces need both: the SMAS has dropped and the skin above it has thinned. Treating only one tends to leave the other looking unfinished — a sharper jawline with tired skin, or firmer skin on a face that still sags.
Both can be done in one visit. The plan is set by layer and area so the same spot is not overloaded, and each device’s shots are written down separately. That combination is the idea behind our Layer Lift.
Shots: why more isn’t better
Shot counts are the easiest number to compare, which is why they are advertised. But Ultherapy lines and Thermage pulses are different units, and the same number of lines spread across the whole face or concentrated on the jawline gives different results.
What matters is where the energy goes. Before we start, your record lists the shot count — for example, 4.5 mm along the jawline and 3.0 mm on the cheeks — so you can see the plan rather than a single total.
Genuine tips and the counter
For Thermage FLX the treatment tip is single-use: it should be new and sealed, opened in front of you, and the system tracks the pulses used from it. Ultherapy transducers are used for a set number of lines, so the thing to check is the device counter before and after your treatment rather than a new seal.
At Taut we write the plan on a paper record before treatment, record the tip and product lot numbers, and check the counter with you when we finish. Our guide to checking shot counts in Korea lists the questions worth asking before you book.
Planning around your flight
Both treatments are usually back to normal the same day, so they fit short trips. Mild swelling, redness or tenderness can last a few days; temporary numbness or bruising is possible but uncommon. Most people fly comfortably the next day.
Avoid saunas, hot baths and alcohol for about a week, and use sunscreen. If you are adding fillers or skin boosters on the same trip, see how long to stay in Seoul — injectables need a little more time before a flight.
When to wait or skip
- Pregnancy — we wait.
- Active infection, an open wound or severe acne in the treatment area — we wait until it settles.
- A pacemaker or another implanted electronic device — tell us before booking; Thermage is not used.
- Metal implants or recent surgery in the area, or recent filler or threads in the same place — tell us so we can check whether, and when, it can be done safely.
- A very slim face with little fat — deep ultrasound is used sparingly, if at all.
Prices depend on the device and shot count; current prices are on the Ultherapy Prime and Thermage FLX pages.
FAQ
Which lasts longer, Ultherapy or Thermage?
Both commonly last about 6–12 months and are repeated around once a year. How long yours lasts depends on your skin, your age and how much the tissue has changed.
Which one hurts more?
Ultherapy is usually felt as brief, deep aches along the jawline and over bone; Thermage as quick bursts of heat with cooling. Both use numbing cream, and we check how you are doing as we go. If you are worried about pain, let us know beforehand. Besides numbing cream and cooling, we can use painkillers, anesthetic injections or sedation, depending on the treatment, to manage pain first.
Can I have Ultherapy and Thermage on the same day?
Yes. We plan them by layer and area so the same spot is not overloaded, and record each device’s shots separately.
When will I see results?
Some people notice firmness straight away, but the main change builds over two to three months as collagen rebuilds.
Can I fly home the day after?
Usually yes. Both are typically back to normal the same day. Tell us your flight time so we can plan any other treatments around it.
How many shots do I need?
It depends on your face, the layer that has changed and the areas treated. We write the shot count on your record before starting; a bigger total is not automatically better.
Is Ultherapy Prime different from Ultherapy?
Ultherapy Prime is the latest version of the Ultherapy system. The principle — micro-focused ultrasound guided by live imaging — is the same.
Do I need a consultation first?
Yes. We check skin thickness, the depth of sagging and facial fat in person before choosing, and we never ask you to decide on the day.
General information, not medical advice. Your plan is set at an in-person consultation; results vary by individual.
